To comply with DC Health regulations, all appropriate health forms must be uploaded to your Sheridan Central account by:
Current students: Monday, August 1, 2022
Incoming students: Monday, August 8, 2022

Required Health Forms
(Each form requires a signature from your child’s health provider.)
- DC Universal Health Certificate
Provides basic information about your child’s health as well as their immunization record. - DC Oral Health Assessment
Provides basic information about your child’s oral health. - Permission to Administer OTC Medication at the School’s Discretion
Permits the Sheridan School nurse, their designatee, and/or authorized Sheridan or Camp Horizons (Mountain Campus) staff to administer any over-the-counter medications to your child if needed.
Supplemental Health Forms: Specific Conditions/Circumstances
(Forms must be completed by your child’s health provider.)
- Prescribed Prescription or OTC Medication Authorization Form
Permits the Sheridan School nurse, their designatee, and/or authorized Sheridan or Camp Horizons (Mountain Campus) staff to administer routine medically prescribed medication during school hours and/or school-related trips. - Self Administration of Emergency Medication
For students who require an epinephrine injector, rescue inhaler and/or insulin and may self-administer the associated medications.
- Asthma Action Plan Form
For students who need a rescue inhaler at school. - Allergy Action Plan Form
For students with food or other allergies and require an epinephrine injector at school. - Diabetes Action Plan Form
For students with diabetes who require insulin monitoring or administration at school. - Seizure Action Plan Form
For students at risk of seizures. - HPV Opt-Out Form
For students who are age-eligible for the HPV vaccine and opt out to receive it.
