Supplemental Health Forms: Specific Conditions/Circumstances (Forms must be completed by your child’s health provider.)
Prescribed Prescription or OTC Medication Authorization Form Permits the Sheridan School nurse, their designatee, and/or authorized Sheridan or Camp Horizons (Mountain Campus) staff to administer routine medically prescribed medication during school hours and/or school-related trips.
Self Administration of Emergency Medication For students who require an epinephrine injector, rescue inhaler and/or insulin and may self-administer the associated medications.
Arrival Time: Students arrive between 8:15-8:25 am. Morning Meetings begin promptly at 8:30 am.
Before Care: We offer complimentary before care from 7:45-8:15 am. Before care is held either on the playground or in the gym (depending on the weather).
Afternoon Dismissal: The school day ends at 3:30 pm (2:30 pm on Wednesdays).
